Research Notes on Advanced Congestive Heart Failure Symptoms


Congestive heart failure (CHF) is functionally categorized into four classes using criteria established by the New York Heart Association and based on the patient’s symptoms and ability to carry out physical activity. Class III patients have marked limitation to their activities and are comfortable when not exerting themselves. Class IV patients, those with advanced heart failure, display severe limitations and are comfortable only while resting.1 Patients with advanced congestive heart failure generally display peripheral edema and pulmonary edema and often experience extreme generalized weakness, dyspnea (difficulty breathing or shortness of breath), orthopnea (difficulty breathing while lying down), and rapid or irregular heartbeat (arrhythmia).23

  • In advanced CHF, nausea and loss of appetite can be caused by fluid accumulation (edema) in the gastrointestinal tract, leading to weight and muscle loss (cardiac cachexia).2
  • Treatment of advanced CHF is aimed at alleviating the symptoms and improving the patient’s comfort level as much as possible.4
